Request for Applications – Partnering for Change: Reducing Infant Mortality in Ohio Communities

Cradle Cincinnati, on behalf of the Ohio Department of Children and Youth (DCY), is issuing this Request for Applications (RFA) to identify and engage partners in select Ohio communities that will work with Cradle Cincinnati to strengthen their infant mortality reduction efforts. Selected applicants will collaborate with Cradle Cincinnati to build leadership capacity, analyze data, engage communities, and implement evidence-based interventions tailored to local needs. This RFA represents an additional opportunity to coordinate infant mortality reduction efforts through innovative, community-driven approaches.  

This project is funded by DCY. Each organization selected for participation will receive $250,000/state fiscal year from the DCY over the planned project period to support project activities. This project will kick off in March 2025 and run through June 2027.

Note that the DCY is restricting this RFA to the following Ohio counties: Cuyahoga, Franklin, Lucas and Montgomery.
