2022 Champions for Change Nominations Open
Hamilton County is filled with passionate individuals from all professions, communities and walks of life.
Help us celebrate those who have done exceptional work fighting for moms and babies over the past year – nominate a Champion for Change!
Nominations are open for the following categories:
Safe Sleep Champion: An individual working to promote safe sleep for babies in Hamilton County. Remember: Alone, on their Back, in a Crib.
Champion for Equity: An individual that recognizes and addresses the disadvantages of marginalized groups and how it leads to health disparities in maternal outcomes.
Champion for Smoke Free Women: An individual who empowers women to quit smoking and/or advocates for smoke free communities.
Champion for Better Care: A nurse, midwife, medical assistant or other provider who cares for women during their pregnancies.
Community Health Worker Champion: A community health worker who walks side-by-side with moms through pregnancy.
Champion for Knowledge: An epidemiologist, researcher or student working to better understand the complexities of infant mortality and preterm birth.
Community Activist Champion: A member of the community that listens, celebrates and actively supports and nurtures their work while keeping moms at the center.
Home Visitor Champion: A home visitor who supports new moms and their families to raise successful thriving children.
